Known issues on the 2008 ES 350. Five recalls are on federal record for this model year. Three of them cover the passenger-side frontal air bag inflator module: in one, an inflator explosion may result in sharp metal fragments striking the driver or other occupants, resulting in serious injury or death; in the other two, an inflator rupture may result in metal fragments striking the vehicle occupants, with the same risk of serious injury or death. A fourth covers the accelerator pedal under vehicle speed control, where a pedal stuck wide open may result in very high vehicle speeds and make the vehicle difficult to stop, which could cause a crash, serious injury or death. The fifth is an equipment recall on the optional all-weather floor mat: if the mat is not secured by itself, or is placed on top of an existing carpeted floor mat, it could move forward and may interfere with the accelerator pedal, and the accelerator may temporarily become stuck in a partially depressed position when returning to the idle position, which may increase the possibility of a crash. THE AWFM INCLUDES TWO GROMMET HOLES AND RETAINING HOOKS (CLIPS) TO SECURE THE MAT TO THE VEHICLE'S CARPET. IF THE AWFM IS NOT SECURED BY ITSELF OR IF IT IS PLACED ON TOP OF AN EXISTING CARPETING FLOOR MAT, THE MAT COULD MOVE FORWARD DURING THE VEHICLE USAGE AND IT MAY INTERFERE WITH THE ACCELERATOR PEDAL.
